To achieve a pregnancy from your molecular level, everything has to be in prefect harmony so that you will achieve a good pregnancy. So for a pregnancy to occur, there are so many things that went right for the pregnancy to happen. So from the time the hormones are released form the brain, they acts on the egg and the egg production starts and they mature and they release. once the egg is released by a technique called as the chemoattraction, the tube will go and pick the egg and once that egg is picked up, the egg will start there for 22 to 24 hours and from the sperm ejaculation into the vagina, the sperm is exposed to the vaginal environment and it should survive in that environment, then it will swim and go to the cervix and into the uterus and then into the tube. For the sperm and the egg to meet should have a patent tubes or open tubes. Once inside the tubes, it will search for the egg by a chemical reaction. After this chemical reaction, the sperm DNA and the egg DNA mixes an di will form an embryo and from that one cell, it will form a 2 cell and from that 2 cell, it will become a 4 cell and then a beautiful embryo is formed inside the tube and after that the tube will push the embryo back into the uterus in one or 2 days, then the embryo will sit into the uterus and develop into blastocyst and the outer shell will break and the embryo comes out and try to find a suitable place in the uterus and it will starts attaching, that is called as implantation and the maternal blood supply establishes. So if in this whole system, even if one link goes down, then the entire procedure will go wrong. So when we do an investigation, then what we do it is not possible to do investigation at every molecular level. So we look at the gross things. So standard test, there is a test called the HSG or by a laparoscopy and do a hormone test and a day today test to see whether a hormone imbalances are there or to see if the egg production we monitor by doing a scan to see if there is hormonal imbalance is there or to see the egg production, we monitor by a scan to see if the ovulation is happening or not or a simple semen test where the count, the motility an the morphology of the sperm. There is a physical test, we do not do a chemical level test, in the older age we do a certain test called as the AMH and it will tell us the egg reserve or how much eggs are thereat reserve at the present. This AMH along with Day 2 day 3 AFC, means Antral Follicular Count. So AMH an AFC is called as the egg reserve test, especially in the cases of older women when they are running to achieve pregnancy after 35 years of age.
