What is PESA/TESA?

PESA, stands for Percutaneous Epididymal Sperm Aspiration while TESA stands for Testicular Sperm Aspiration. It involves direct aspiration of the sperm from different structures in the male anatomy.

PESA is a minimally invasive procedure used to retrieve sperm directly from the epididymis. The epididymis is a small, coiled tube located behind each testicle where sperm mature and are stored.

TESA, on the other hand, involves extracting sperm from the testicular tissue.

Understanding Azoospermia.

Azoospermia is a medical condition where a man’s ejaculate contains no sperm. Scientifically, azoospermia affects approximately 1% of the male population and is a significant cause of male infertility.

It is classified into two main types:

Obstructive Azoospermia:

This occurs when sperm is produced in the testicles but is blocked from reaching the ejaculate due to obstructions in the male reproductive tract. The causes can include previous vasectomy, congenital absence of the vas deferens, infections, or scar tissue from surgery or injury.

Non-Obstructive Azoospermia:

This type occurs due to a disorder with sperm production in the testicles themselves. The causes may include genetic conditions, hormonal imbalances, or environmental factors such as exposure to toxins or radiation.

Choosing PESA/TESA

PESA is applied when there is a blockage in the male reproductive tract, specifically in cases of obstructive azoospermia. This blockage prevents sperm from reaching the ejaculate, despite normal production in the testicles.

TESA is utilized in cases of non-obstructive azoospermia, where sperm production is impaired within the testicles themselves. It is also used when PESA fails to yield sufficient viable sperm, providing a direct approach to sperm retrieval from the source.

PESA vs TESA: The Procedure

PESA is a quick outpatient procedure, typically completed in less than 30 minutes, allowing patients to return home the same day. Recovery is generally fast, with only mild discomfort and swelling, which usually subsides within a few days.

TESA is a bit different and more complex. Sperm collected through TESA may not be as mature as those collected through PESA, but they can still be used for ICSI, especially in difficult cases where sperm are hard to find. It is also a quick outpatient procedure, with recovery similar to PESA, involving minimal discomfort and a swift return to normal activities.
